Eligibility/Requirement
- Must receive his or her salary through the Controller and Accountant General Department
- Must have affordability
- Must provide a mandate form
- Must provide current month pay slip
- Must provide one passport-sized picture.
- Undertaking or Authority note from immediate supervisor (signed Authority note Form)
